R.A PETSTYLIST CLIENT AGREEMENT & LIABILITY WAIVER
By booking an appointment with R.A PetStylist, you acknowledge and agree to the following terms:
- I confirm that I am the legal owner or authorized guardian of the pet(s) presented for grooming.
- I certify that my pet(s) are in suitable health for grooming and free of contagious conditions. I agree to disclose any medical, behavioral, or psychological conditions.
- I understand that grooming involves inherent risks including minor cuts, skin irritation, stress, or behavioral reactions.
- I release and hold harmless R.A PetStylist, its owner, and staff from any liability related to injuries or conditions that may arise during or after grooming, especially in cases involving pre-existing or undisclosed conditions.
- I acknowledge that aggressive or unsafe behavior may result in immediate termination of the service.
- I understand that severely matted coats may require shaving and may expose pre-existing skin conditions.
- I agree to pay for services in full upon completion and accept any additional fees if applicable.
- I agree to provide at least 24 hours notice for cancellations or rescheduling.
- I understand that R.A PetStylist provides mobile grooming services at the client's home, ensuring professional quality and convenience.
- I authorize R.A PetStylist to act in the best interest of my pet's health and comfort during the grooming process.
By signing below, I confirm that I have read, understood, and agreed to all R.A PetStylist policies and terms.
